What is a proxy card?

A proxy card allows someone to borrow materials on your behalf. Although the proxy carries the card, you are responsible for all materials borrowed on the card.

How do I appoint a proxy?

The following information must be supplied in writing, preferably on department letterhead, at any University Libraries' Circulation Desk:

  1. Responsible person's name
  2. Responsible person's UB Library Card barcode number (begins with 29072; found on lower left corner)
  3. Responsible person's department
  4. Responsible person's campus or home address
  5. Responsible person's e-mail address
  6. Responsible person's telephone number
  7. Proxy person's name
  8. Proxy expiration date

Who receives the proxy card?

The proxy card will be issued to the responsible person and must be signed by both the responsible person and the proxy.

Any instructions for using the card?

The proxy must show photo ID when using the card.
